Before Your First Visit
For your first visit, please take time to complete the New Patient Health History form, which is under the "Patient Resources" tab above. By completing this form prior to your arrival, we can begin your first visit without delay. Also be aware of your insurance coverage, if you have coverage. If you do have orthodontic coverage, please bring the needed paperwork so that we may assist you in getting your orthodontic benefits.

We look forward to meeting you on your first visit to our office. During this visit, we will help you get acquainted with our office and learn about your orthodontic treatment. Stop by and say hello to Tina, our receptionist, before going in for your appointment. She can help you with any questions you may have regarding your new patient form and will also make a copy of your insurance card to keep on record.Our treatment coordinator, Teri, will take you to our consultation room, where you will meet with Dr. Sydlewski. We pride ourselves on educating our patients each step of the way, and the initial exam is the start of that process.
Prior to meeting with Dr. Sydlewski we take photos and any necessary x-rays to determine if treatment is necessary.

- · Treatment is indicated; however, Dr. Sydlewski feels that treatment should be delayed until appropriate growth, tooth eruption, or other factors are monitored and reconciled. In this case, you will be placed on observation and we will closely monitor your condition through regular examinations.
After careful examination, Dr. Sydlewski will discuss one of the following with you.
If Dr. Sydlewski feels you are ready for treatment, we will get started on complete diagnostic records. Diagnostic records are made to determine the best course of treatment and typically include a medical/dental history, clinical examination, plaster study models of the teeth, photos of the patient's face and teeth, and X-rays. This information is used to develop a custom treatment plan for each patient. These records are also helpful in tracking the progress of treatment as the teeth move under orthodontic care. We will provide a complete clinical diagnosis regarding the nature of the problem, a written summary of the findings and a detailed plan that outlines your treatment, goals and estimated fees. We encourage questions from both the patient and the parent, and as such, we provide ample time to answer your questions.
Dr. Sydlewski has set aside one hour for your visit with NO FEE for this initial exam/consultation. So that we may continue offering this as a COURTESY visit, please contact the office at least 24 hours in advance should you need to reschedule.
Initial Placement of Braces
The appointment when you get your appliances (braces, expanders, etc.) usually takes from one to two hours. We do these appointments in the morning, when it is quiet; to allow you to have Dr. Sydlewski’s undivided attention.
Scheduling Regular Appointments
Patients are seen by appointment. We achieve quality care by budgeting our time wisely, so we do our best to stay on schedule. We know that your time is valuable, too, and we try to keep your wait to a minimum.
Our use of cutting edge technology allows patients to be seen at a 4 to 8 week interval with an average adjustment appointment length of 20 to 30 minutes. We schedule longer appointments in the morning and mid-morning and shorter appointments after school. By doing this, we do not reserve an entire afternoon for one or two patients but rather accommodate as many families as possible for after-school appointments.
Because we serve such a range of orthodontic patients, and accommodate the scheduling needs of our patients while delivering the best orthodontic care, we have developed a rotating scheduling. The idea is that you can have, for example, an after school new patient appointment if you want one, or one first thing in the morning if you choose. There will be a different day each week to fit your needs. Our effort is to make our practice convenient for you, since our goal after all is to serve you.
If you cannot keep an appointment, please notify us immediately. Cancel only if it is an emergency. It is difficult to reschedule you since most appointments are made 4 to 8 weeks in advance.
Referrals and Appointments
We love referrals! One of the best compliments that you can give us is to refer someone to our office. We greatly appreciate your confidence in our ability to help others evaluate their orthodontic concerns. Some people are worried that a referral needs to come from their dentist. While we certainly receive referrals from general dentists, it is also appropriate for our patients or their parents to refer others directly to our office. We provide all referrals the proper information to understand their orthodontic issues and the potential for treatment.
Financial and Payment Information
Financial considerations should not be an obstacle to a beautiful smile. When it comes to your or your child's face, there is just too much at stake to base your decision on the lowest possible price. We are dedicated to making certain that you can afford the very best! You do not have to settle for less than you want or deserve.

We thoroughly explain your payment options, so that we can accommodate your needs.
Our practice offers a wide range of payment options. Financing with no initial payment may often be arranged. We will work out a plan suited for your individual needs. For those who pay in full at the beginning of treatment we offer a discount.
For your convenience, we accept Visa and MasterCard and offer no-interest financing for the length of the treatment. We offer a family courtesy, as well as a bookkeeping courtesy for payment in full.

Cost of Treatment
Cost of treatment is based on severity of the patient's problem, time in treatment, and quality of care. This will be discussed at the consultation appointment prior to initiating any treatment, thus making the most of the valuable time you have set aside from your busy schedule to meet with us. You will be able to discuss fees and payment options before treatment begins. We offer many payment options (including a no-interest, monthly payment plan) designed to suit different budgets. We also accept assignment from most insurance plans, and file the necessary papers to the insurance company. We work hard to make orthodontics affordable.
